You are required to keep following original documents/certificates ready for joining at the college for document verification & checking of eligibility:

i. NEET-UG-2024 Result card .
ii. Matric (10th Class Certificate) for verification of age.
iii. 10+1 Detailed Marks card for verification that candidate has studied Physics, Chemistry, Biology and English.
iv. 10+2 Detailed Marks Card for verification of Eligibility as per Punjab Govt. notification and NEET-UG-2024 Bulletin.
v. Migration Certificate
vi. Birth Certificate of Punjab.
vii. Certificate of Permanent Resident of Punjab as per clause no. 11 of notification.
viii. Exemption certificate (if any).
ix. Compulsory Sworn Declaration by the parent/guardian (in case of minor child) or by the candidate (in case of major child)-as per page
no. 58 of prospectus.
x. Gap undertaking (if any).
xi. All other documents/annexures required as per MBBS/BDS – 2024 Prospectus and NEET-UG-2024 Bulletin.
xii. Original Receipt of Deposition of Tuition Fee.
xiii. One of the identification proofs(ID Proof) i.e. PAN Card, Driving License, Voter ID, Passport or Aadhaar Card.
xiv. Reserved Category Certificate issued by the respective Authorities as described in the Prospectus. The category certificate should fulfill
the terms and conditions as per format available in the Prospectus.
xv. PWD category certificate as issued by the designated centre(s) as notified by Medical Counselling Committee(MCC) in Information
Bulletin.
xvi. Relevant documents/proofs of Minority Quota in case selected in Sikh/Christian Minority Quota as required by the allotted college.
